I (Jyoti Prakash) requested for change in my name in 12th certificate examination held on march 2019. Original certificate which have incorrect name and photo copy of matric certificate attached and forwarded by study centre through Post office. Sir when I receive my original certificate with correct name. Incorrect name is JYOTI PARKASH.
